Fix sport feed quality-gate errors losing their diagnostic message
UserInputException hardcodes its message to "Parameter X is missing or
invalid" regardless of what's passed as $type, so the actual
SportFeedQualityGate diagnosis was silently discarded before it ever
reached the sync log or error log - only this generic, useless string
was recorded. Switched to plain RuntimeExceptions so the real reason a
check/provision attempt failed is actually visible.

Fix demo provisioning: stale app cache, wrong DB match, orphaned DB cleanup
The demo creation cron never completed because updateDemoAppSettings()
only deleted the v2-named application cache file, while this WCF version
generates a v3-named one - the stale cache kept redirecting health checks
to the original template's domain, which then dropped Basic-Auth creds on
the cross-domain redirect and failed with 401.
Also fixes createNewDemo() resolving the wrong (stale, same-comment)
database on retries after a prior failed attempt - now reads the dbName
straight from add_database's own response instead of matching by comment
across all databases. Retries the mysql import a few times to tolerate
freshly created KAS credentials taking a moment to propagate, uses a more
broadly compatible {SHA} hash for the demo's .htpasswd, and stops
demos-pending-deletion from blocking the cleanupDatabases()/
cleanupFileSystem() orphan safety net.
